Our Referral Requirements

When referring a patient to Alaska Regional Hospital’s Acute Inpatient Rehabilitation Unit, please provide as much of the following information as possible:

  • Demographics, including: patient’s contact information, social security number, insurance provider and contact information
  • Reason for admission
  • Medical history and physical
  • Progress notes
  • Social services assessment
  • Discharge planning notes that identify a discharge plan and include the name of the decision maker, if applicable
  • Physical therapy (PT), occupational therapy (OT), speech-language pathology (SLP) evaluations with recommendations and goals, along with notes from the most recent three days of therapy
  • Mental status (sitter, restraints)
  • Recent complete blood count (CBC), chemistry panel and other relevant labs
  • Diagnostics (imaging report such as MRI, CT, Doppler, TTE, EKG)
  • Vital Signs (current and previous 24 hours)
  • Height, weight and nutritional status
  • Wounds
  • Infections
  • Isolation precautions
  • Pending appointments or additional therapies
  • Status of applications for long-term care/assistance
